How YouTube's Ad Placement Algorithm Got It So Wrong

Major consumer brands have pulled out of advertising with YouTube after it was discovered that some of their ads were running alongside offensive content including video from terrorism-affiliated groups. Google, which owns YouTube, places ads on videos in part through an algorithm-powered system. So…what went wrong?
