The commission looking into the prisoner abuse scandal in Iraq has released its report, concluding, "There was chaos at Abu Ghraib." Former Defense Secretary James Schlesinger says the military police at the prison were "undertrained for detention operations," there was a low ratio of military police to the number of inmates and the abuses were "not limited to a few individuals."
